Completed New Patient Registration forms above may be initially returned electronically to the Practice generic email address: gram.ellonadministrator@nhs.scot In the event that you incur file access issues opening the Application Form, which is an NHS Scotland protected file, we suggest using Internet Explorer browser. This form cannot be downloaded for security, but must be completed online and printed, with scanned or paper copy submission return.
You will be asked to complete a registration form for each person registering with the Practice. In addition, all patients will be asked to complete a new patient questionnaire. This enables us to provide medical care in the interim period, whilst your medical records are transferred from your old Practice to new.
If you move to an address outside of the Practice area you will require to register with a new local Practice as soon as you move. We can provide information regarding how to find a new Practice.
You and Your Doctor
Patients are registered with the Practice, not with an individual GP; you can at any time express a preference for a particular doctor, for either or all of your medical needs or on a case by case basis. However, not all of the doctors based within the Practice provide all services , furthermore specific requested doctors may not be always be available on the day requested.
