• *This questionnaire applies only to patients already taking HRT who require repeat prescription/review. Please call the practice to speak with a clinician if you wish to discuss starting HRT*.
• Unless you are contacted with any further review/repeat query, your prescription will be available from the pharmacy within 5 working days.  Blood pressure reading can be provided by home monitor or visiting a pharmacy that has a self check BP machine.

Contraception Review Questionnaire

This questionnaire  applies only to patients already taking the Contraceptive Pill /Contraceptive Patch who require repeat prescription/review . Please call the practice to speak with a clinician if you wish to discuss starting contraception.
